There are a number ways in which we can directly assist with socialising & community access for the elderly living in their own home:

 

 

Socialising & community access as part of a Home Care Package

The Home Care Packages Service provides general, Veteran Affairs & Housing Linked Care packages. These are Commonwealth funded programmes that provide hostel-level care in the home. A “Care Package” is a combination of services provided to an individual client and can include:

– Access to Socialisation

– Dressing/Undressing

– Laundry Assistance/Ironing

– Lawn Mowing

– Light Housework

– Light Maintenance

– Meal Preparation

– Medication Supervision

– Pet Care

– Shopping

– Showering/Bathing

– Transport to Appointments (by arrangement)

ELIGIBILITY

Each person needs to be assessed by the Aged Care Assessment Team (ACAT) before being eligible for a package of services. Anyone can refer a prospective client to the Referral Information Centre (Phone: (02) 4925 7990), to be assessed by ACAT. Areas covered by our service are Newcastle and Eastern Lake Macquarie LGA’s.

COST

The Government has set 17.5% of Aged Care Pension as the basic standard fee. The actual fee you pay may be higher or lower than this depending on your circumstances. Our Coordinator can explain the details and ensure that you get the help you need.

Socialising & community access under the Home & Community Care program

The Social Support Service has caring volunteers who can assist you with the following:

  • • Visits, phone calls and social activities with an individual volunteer
  • • Bus trips
  • • Group social activities

Inner Newcastle area _ (Adamstown; Adamstown Heights (only Newcastle LGA part); Bar Beach; Broadmeadow; Carrington; Cooks Hill; Fern Bay; Georgetown; Hamilton; Islington; Kooragang; Kotara (only Newcastle LGA part); Maryville; Mayfield; Mayfield West; Merewether; Newcastle; Stockton; The Hill; The Junction; Tighes Hill; Warabrook; Waratah; and Wickham

Wallsend area

Western Lake Macquarie area (encompassing Seahampton to the north, Wangi Wangi to the south, Glendale to the east and Wakefield/Awaba to the west).
